CCC Youth Ministry Liability Waiver & Consent Form
I, the undersigned parent/legal guardian, give permission for my child to participate in Sunday School and/or Youth Ministry activities organized by CCC. I understand that these activities may include teaching sessions, worship, group discussions, games, outings, and special events.
